  4. Project Description on Experience to Date

    1. Project Description

      1. Problem:

        The purpose of the grant is to reduce loss to follow up and loss to follow through of children with confirmed hearing loss birth to age 3 and to monitor child performance while receiving early intervention services so that optimal developmental outcomes may be achieved by transition to school-age.

      2. Goals and Objectives: List up to 5 major goals and time-framed objectives per goal for the project.

        Goal 1: Increase amount and quality of data reported for newborns screened for hearing loss by age 1 month
        Objective 1: By December 2005, fully integrate newborn hearing screening into the Florida Newborn Screening Program.
        Objective 2: By January 2006 provide regular feedback to birth hospitals regarding their newborn hearing screening accuracy.
        Objective 3: Provide ongoing education and support to birth facilities to increase effectiveness and reduce referral rates.
         
        Goal 2: Increase the number of children with hearing status reported by 3 months of age.
        Objective 1: Distribute standard of care for newborns referred from hearing screening to physicians.
        Objective 2: Develop Audiology Centers of Expertise (ACE) to which all infants referred from newborn hearing screening will be directed.
        Objective 3: Reduce loss to follow up and loss to follow through via parent-to-parent support.
         
        Goal 3: Increase number of children receiving early intervention services by 6 months age
        Objective 1: Increase the numbers of children with confirmed hearing loss referred by the diagnosing audiologist to the Early Steps Program within two working days of confirmation of hearing loss.
        Objective 2: Assure that referrals of infants and toddlers with confirmed hearing loss have been received by local Early Steps offices.
        Objective 3: Provide parent-to-parent support to families of infants with confirmed hearing loss.
         
        Goal 4: Modify processes and activities as needed to improve EHDI effectiveness
        Objective 1: Use results of data analysis as a foundation for determining what changes need to be made to the activities.
        Objective 2: Determine the need for training initiatives or educational materials to improve EHDI effectiveness. Provide educational materials or training to targeted groups as appropriate to address identified areas of need.
         
      3. Activities/Methodology undertaken to meet project goals:

        •Receive hearing screening results from State Laboratory on newborns born in Florida •Send hearing screen and metabolic results to birth facilities and newborn's physician of record •Report hearing screen and hearing referral rates by birth facility in newsletter •Include the number of infants with confirmed hearing loss in newsletter •Provide onsite technical assistance to birth facilities •Reduce referral rate in hospitals with rates greater than 3.5% •Include standard of care language for newborns referred from newborn hearing screening on physician results letter •Provide hearing screening information and technical assistance to physicians •Determine how Audiology Centers of Expertise for infant/toddler audiology can be established in Florida •Implement the Hearing Education Link via Parent Support (HELPS) concept •Continue to educate audiologists about the need to refer children with confirmed hearing loss to both early intervention and the EHDI program •Establish procedures for HELPS providers to work with families with confirmed hearing loss •Explore the feasibility of modifying the HELPS program to a Hands & Voices Chapter or a Beginnings model •Collect, analyze and report data to show strengths and weakness of program; modify program as necessary •Develop and provide educational materials and training as appropriate to address areas of need

      4. The first three Healthy People 2010 objectives which this project addresses are listed below.

        1. Related to Goal 1: Improve access to comprehensive, high-quality health care services (Objectives 1.1- 1.16).
        2. Related to Objective 16.23. Increase the proportion of Territories and States that have service systems for Children with Special Health Care Needs to 100 percent.
        3. Related to Objective 16.23: Increase the proportion of States and jurisdictions that have service systems for children with or at risk for chronic and disabling conditions as required by Public Law 101-239.

      5. Coordination: List the State, local health agencies or other organizations involved project and their roles.

    The project's purpose is treduce loss to follow up of children failing newborn hearing screening and identify hearing loss by 3 months. Only about 30% of expected children with hearing loss receive Part C early intervention. The project wants to improve hearing screening, hearing loss determination, and early intervention enrollement.
